New patients

Would you like to register as a new patient? You can do so if you live in Tolbert or Boerakker and your current GP is located outside the Leek/Tolbert region. Below you will find answers to some frequently asked questions.

  • You can register with the practice by visiting the reception desk. Unable to visit in person? Then please call the assistant.

    It is important that you identify yourself when registering. Therefore, please visit the practice during opening hours, fill out a registration form, and show a valid form of identification to the assistant. Are you registering one or more family members? Please bring their (copied) IDs. You can find the practice’s opening hours on our homepage.

    When registering, you can change your pharmacy to BENU Apotheek Tolbert, which is located in our building. Are you switching pharmacies? Then please also register at the BENU Tolbert reception desk.

  • Unfortunately, we are currently unable to accept patients from surrounding villages.

    Huisartsenpraktijk Poort van Leek (formerly Huisartsenpraktijk Wolveschans) is unfortunately temporarily closed to new patients. The same applies to Huisartsen Groepspraktijk Leek.

    Do you live in Zevenhuizen or in the Oostindië neighbourhood in Leek? Then you are welcome to register at Huisartsenpraktijk Zevenhuizen.

    If you live in Nuis or Niebert, Huisartspraktijk F.G. Schuch in Marum is open for your registration.

  • Are you moving to the region and is the rest of your family or household already registered with us? In that case, you can also register as a new patient. We strive for the best care, and we find it wise to register the whole family within the same practice.

  • When you register as a new patient, we do not automatically schedule an introductory meeting. However, should you feel the need for one, you are of course very welcome to attend our consultation hours.

    Please note: it is important that we already have your file from your previous GP at the time of the introductory meeting. Transferring the file usually takes one to two weeks. Therefore, do not schedule an introductory meeting too soon after registering.
